The Institute for Safe Medical Practices has released the most frequently misinterpreted abbreviations and dose designations. The list was compiled from the Medication Error Reporting Program. Certain abbreviations have been so error-prone that they have been put on the “do-not-use list” by the Joint Commission on Accreditation of Healthcare Organizations.
